Skin barrier protection could be the new skin care

For millions of people worldwide, skin conditions often represent far more than an occasional skin irritation. It is often a chronic condition that can affect sleep, concentration, mental well-being and daily quality of life. Estimates from the British Journal of Dermatology suggest that atopic dermatitis, the most common form of eczema, affects over 200 million people globally, making it one of the most prevalent inflammatory skin conditions. 

New research from the National Eczema Association says that in the United States alone, more than 31 million people live with some form of eczema. For many patients, managing the condition becomes a continuous process of applying treatments, avoiding triggers and attempting to control flare-ups that can return despite ongoing care. As the prevalence of chronic skin conditions continues to rise, researchers and clinicians are increasingly examining whether existing approaches fully address the underlying challenges faced by patients.

For decades, eczema treatment has largely focused on reducing inflammation. Studies by the National Institutes of Health have shown that topical corticosteroids have remained a cornerstone of care, while newer immunomodulating therapies and biologic medications have expanded treatment options by targeting specific inflammatory pathways involved in disease activity. However, literature published by the National Library of Medicine notes that dermatology research has increasingly emphasized another fundamental component of skin health: the integrity of the skin barrier. The outermost layer of the skin serves as a protective defense system, helping maintain moisture balance while limiting exposure to allergens, microbes and environmental irritants.

Studies on atopic dermatitis published in the Journal of Investigative Dermatology have identified epidermal barrier dysfunction as a hallmark of the condition. The skin’s outermost layer plays a critical role in maintaining moisture balance and protecting against environmental exposure. When this barrier becomes compromised, increased water loss and greater permeability to external triggers can create further vulnerability to irritation and inflammation.

This growing understanding has prompted broader discussion within dermatology about whether protecting vulnerable skin should play a larger role alongside existing approaches that target inflammation. For AJ Singhal, founder of Kalm Wellness, the answer lies in addressing a part of the disease cycle that he believes has been overlooked: preventing further damage while the skin attempts to recover.

When his six-year-old daughter struggled with severe eczema, Singhal saw firsthand how difficult it was to manage a condition where treatment alone did not always prevent continued scratching and damage, especially during sleep. Traditional approaches involving creams, wraps and repeated applications became difficult to maintain, leading him to explore whether a simpler protective solution could help interrupt the cycle. His work eventually led to the development of Kalm Patch, a patch-based approach designed around the idea that protecting compromised skin may be an essential part of supporting its natural recovery process.

“Skin protection is the missing piece in the itch and inflammation conversation,” Singhal says. “When the skin barrier is already damaged, continuing to expose it to scratching, friction and irritants makes it harder for the skin to recover. Protecting that barrier gives the skin a better opportunity to heal itself.”

Singhal calls the approach a “triple benefit” model: protecting against scratching, shielding vulnerable skin from external irritants such as pollen and bacteria, and reducing moisture loss from the skin’s surface. The concept addresses what Singhal observed as the “scratch-damage cycle.” “When itchy skin triggers scratching, the physical disruption can further weaken the barrier, leading to more irritation and additional itching. The cycle can become especially challenging for children, whose scratching often occurs unconsciously during sleep,” he explains.

Singhal’s own experience highlighted the limitations of traditional routines. After over-the-counter products failed to provide enough relief for his daughter, prescribed topical steroids became part of the treatment process. However, the continued scratching remained a challenge.

“We tried everything people typically try,” Singhal says. “There were creams, steroid treatments, wrapping the skin with plastic wrap, then gauze and tape. It was time-consuming and difficult to maintain. I kept thinking there had to be a simpler way to protect the skin while also soothing it.”

While patches have become widely accepted in skincare through products such as acne patches, Singhal believes the same delivery model has broader potential for irritated and vulnerable skin. “The acne patch helped change how people think about treating skin,” he says. “For years, people questioned whether patches would become mainstream. Now they are a familiar part of skincare routines. The same idea of protecting an area and allowing it to recover can apply to other skin challenges.”

Another factor Singhal highlights is controlled delivery. Traditional creams can vary significantly in how much product a person applies, creating challenges around consistent dosing. The patch format is designed to provide a fixed amount of its active compound with gradual release over time.

“The best innovations often come from solving a problem people experience every day,” he says. “The goal was simple. Create something that makes people’s lives easier. Peel, stick and allow the skin to recover.”

As consumer interest grows around science-based, non-prescription approaches to managing chronic skin concerns, Singhal believes the industry is entering a new phase. The next generation of skincare may involve fewer complicated routines and more emphasis on supporting the body’s own repair mechanisms.

“The future of skincare is about understanding that protection is part of healing,” Singhal says. “When we stop repeatedly damaging the skin and give it the environment it needs to recover, we change the entire conversation around care.”
